Steve Russell\/Toronto Star<\/em><\/p>\n<p>Internal talks between the Ford and Trudeau governments over supports for the long-term care sector have not included the aim of reducing for-profit facilities, the premier\u2019s office says \u2014 as recent analyses have revealed higher death rates in for-profit homes.<\/p>\n<p class=\"p1\">Ontario Premier Doug Ford appeared to indicate interest in speaking with the feds about the proliferation of privately-owned long-term care homes last week.\u00a0Asked by a reporter whether he would support, in some capacity, ending privatized care, Ford noted that Ontario couldn\u2019t currently afford to do so on their own.<\/p>\n<p class=\"p1\"><span class=\"s1\">\u201cWe aren\u2019t in the financial spot to be able to fund the whole system, but I\u2019d love to sit down, and I\u2019ve mentioned this to the Prime Minister and Deputy Prime Minister, we need their support,\u201d Ford replied, during a May 6 press briefing. But on Monday, premier\u2019s office spokesperson Ivana Yelich told iPolitics that Ontario\u2019s discussions with the feds about aid hadn\u2019t veered in that direction. <\/span><\/p>\n<p class=\"p4\"><span class=\"s1\">\u201cIt doesn\u2019t necessarily have anything to do with the province going and looking at eliminating all private homes,\u201d Yelich said of their talks. \u201cIt\u2019s not really a conversation we\u2019re having right now, at all.\u201d<\/span><\/p>\n<p class=\"p4\"><span class=\"s1\">Yelich pointed out that for-profit and not-for-profit homes in Ontario have been hit by COVID-19 outbreaks at comparable rates \u2014 a statement that was bolstered by a recent breakdown of public long-term care data <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.thestar.com\/business\/2020\/05\/08\/for-profit-nursing-homes-have-four-times-as-many-covid-19-deaths-as-city-run-homes-star-analysis-finds.html\">in the Toronto Star<\/a><\/span>. But recent analyses by news outlets and health groups have shown a steeper death rate in for-profits with outbreaks than in not-for-profits or municipal homes.<\/span><\/p>\n<p class=\"p4\"><span class=\"s1\">An Ontario Health Coalition assessment of 93 homes with COVID-19 outbreaks recently found that for-profit homes had a 9-per-cent fatality rate, versus a death rate of 5.3 per cent in non-profits and 3.6 per cent in homes owned by Ontario\u2019s municipalities. <\/span><\/p>\n<p class=\"p4\"><span class=\"s1\">The coalition\u2019s <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/shalom-schachter-death-rates-in-ltc-by-ownership.pdf\">published findings<\/a><\/span> were bolstered by breakdowns from news outlets like <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.qpbriefing.com\/2020\/05\/07\/death-rates-at-for-profit-nursing-homes-significantly-higher-than-non-profits\/\">QP Briefing<\/a><\/span> and <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.thestar.com\/business\/2020\/05\/08\/for-profit-nursing-homes-have-four-times-as-many-covid-19-deaths-as-city-run-homes-star-analysis-finds.html\">the Toronto Star<\/a><\/span>, with the latter finding that during outbreaks, residents of for-profit homes were roughly twice as likely to catch the novel coronavirus and die than those in non-profits, and roughly four times as likely to die from the virus as residents in municipally owned homes.\u00a0<\/span><\/p>\n<p>OHC also found that death rates had declined in municipal homes since April 28, but increased in both for-profits and not-for-profits \u2014 with for-profits\u2019 death rates increasing faster out of the two.<\/p>\n<p><span class=\"s1\">Despite the recent analyses, the premier\u2019s office says it will await a post-pandemic review to determine where key vulnerabilities lay. <\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">\u201cThere are so many factors at play, and we have to figure out what exactly happened in some of these homes. Is it the staffing shortage issue? Is it the size of the home?\u201d Yelich said. \u201cI\u2019m not saying there aren\u2019t issues, but there\u2019s likely issues for a number of reasons \u2026 and those are what we\u2019ll really have to zero in on, when this is all done.\u201d<\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">Private ownership of nursing homes expanded in Ontario under the administration of former PC premier Mike Harris. In 2017, a team of researchers from academic institutions across the globe \u2014 including York University in Toronto \u2014 <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28634428\">honed in on several policy changes<\/a><\/span> made in Ontario during the mid-1990s, which they said favoured the growth of for-profit home chains in the province.<\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">The changes they identified included accepting low bids for contracts, publicly financed capital funding, legislation eliminating minimum staffing standards, and a revised payment system that, per the team of researchers, allowed companies to maintain profits.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Earlier still, a requirement for nursing homes to be licensed in the 60s and extension of hospital insurance to homes in the 70s, spurred reliance on for-profit homes and chains, the researchers found \u2014 due to restraints on government funding, and the government\u2019s limited role in quality \u201cregulation and enforcement.\u201d<\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">As of last year, the Ontario Long Term Care Association \u2014 which represents the full slate of provincial operators, including both private and public \u2014 said 58 per cent of Ontario\u2019s homes were privately owned, 24 per cent non-profits or charities, and 16 per cent were owned by municipalities. Another 2 per cent of long-term care was via facilities like hospitals. \u201cMany homes are part of multi-home organizations,\u201d the association noted in <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.oltca.com\/OLTCA\/Documents\/Reports\/TILTC2019web.pdf\">a 2019 report<\/a><\/span>. <\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">The association\u2019s report claimed there were no differences in quality, on average, between privately owned and not-for-profit homes \u2014 both of which they said scored higher for quality of care than municipal homes. On average, the group said smaller homes had \u201csignificantly\u201d lower scores for care quality, and surmised that the cause could be connected to differences in staffing levels.<\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">A study of 640 long-term care facilities in Ontario in the early 2010s, which was <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.jamda.com\/article\/S1525-8610(15)00414-4\/abstract?cc=y=\">published in the Journal of the American Medical Directors Association in 2015<\/a><\/span>, found that for-profit facilities had \u201csignificantly higher\u201d rates of both mortality and hospital admissions than their counterparts. The lead author <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.thestar.com\/news\/canada\/2015\/10\/07\/death-rates-higher-in-for-profit-nursing-homes-report-says.html\">told the Star<\/a><\/span> at the time that, while their work hadn\u2019t addressed the cause for that discrepancy, other research had identified the staffing levels issue.\u00a0<\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">Ontario NDP leader Andrea Horwath recently called for an examination of long-term care ownership and COVID-19 via a public inquiry. \u201cI oppose profits being made by corporations in long-term care,\u201d Horwath said in a statement, in which she vouched for a system of community-based not-profits and public care.\u00a0<\/span><\/p>\n<p class=\"p9\"><span class=\"s1\">Calls to take long-term care out of private hands have also echoed out at the national level in recent days. The Canadian Labour Congress was among them, <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.theglobeandmail.com\/politics\/article-canadian-labour-congress-calls-for-end-to-privately-owned-long-term\/\">per the Globe and Mail<\/a><\/span>, and federal NDP leader Jagmeet Singh <a href=\"https:\/\/www.ctvnews.ca\/politics\/canada-must-put-an-end-to-privately-owned-long-term-care-homes-singh-1.4931754\"><span style=\"color: #3366ff;\">threw his weight behind the call to CTV.<\/span><\/a><\/span><\/p>\n<p class=\"p11\"><span class=\"s1\">\u201cWe absolutely need to look at developing a national standard, a care guarantee and then using the Canada Health Act to bring that into the public realm,\u201d Singh said in the interview this weekend.<\/span><\/p>\n<p class=\"p11\"><span class=\"s1\">\u201cThe for-profit system has failed, and long-term care homes in general have failed seniors, and we need to do a lot better.\u201d<\/span><\/p>\n<p><span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/ipolitics.ca\/2020\/05\/11\/ontarios-call-to-ottawa-for-long-term-care-support-doesnt-aim-to-reduce-private-ownership-fords-office\/\">Click here for original article<\/a><\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>(May 11, 2020) By:\u00a0Victoria Gibson,\u00a0IPolitics Premier Doug Ford at his daily COVID-19 press conference, at Queen&#8217;s Park in Toronto.
