When you see a doctor or visit the hospital for medically necessary services using your OHIP card, you are using Public Medicare. Under Public Medicare, all Canadian residents can receive medically necessary hospital and physician services based on their medical need without user fees or charges. Health care is provided without cost based on our medical need, paid in our taxes, rather than based on our ability to pay or our wealth. <\/span><span lang=\"EN-CA\">Public Medicare enjoys a high level of support from Canadians. It is a source of pride for many, but years of relentless cuts and governments pushing privatization have contributed to problems in Medicare. The solution is not to privatize but to plan well, reorganize care <u>in the public system<\/u>, and restore and rebuild services that have been cut and downsized too far.<\/span><\/div>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h4>What is it like to not have Medicare?<\/h4>\n<ul>\n<li>Before Public Medicare, Canadians had no choice but to forego medical treatment because they did not have the money to pay for care, leading to suffering and death. Helen Heeney\u2019s book <em>Life Before Medicare<\/em> tells the story of a young woman with cancer who refused pain medication because it would bankrupt her family. For two months, she had her husband lock her in their home when he left for work so that no one could enter to help her when they heard her screaming in pain. She did not want to bankrupt her family as she was dying.<\/li>\n<li>In the United States where private insurance and for-profit delivery of health care services are widespread, health care costs are almost <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.oecd.org\/en\/data\/indicators\/health-spending.html?oecdcontrol-00b22b2429-var3=2022\">double<\/a><\/span> per person those in Canada.<\/li>\n<li><span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/medicare4all.org\/wp-content\/uploads\/MFA_and_Racial_Justice.pdf\">More than 26 million Americans did not have health insurance in 2023<\/a><\/span> and <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/medicare4all.org\/wp-content\/uploads\/MFA_and_Racial_Justice.pdf\">38% of residents in the United States delayed getting medical treatment<\/a><\/span> because they could not afford it.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<div>\n<h4>Achieving Public Medicare &amp; the Canada Health Act<\/h4>\n<p><em>The printable version of the fact sheet (linked below) presents the following events in a timeline.<\/em><\/p>\n<p><strong>1947:<\/strong> <a href=\"https:\/\/esask.uregina.ca\/entry\/hospital_services_plan.html\"><span style=\"color: #3366ff;\">The first public hospital insurance plan<\/span><\/a> in Canada was introduced by the Saskatchewan government led by Tommy Douglas.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>1957: <\/strong>The federal government, with Paul Martin Sr. as Health &amp; Welfare Minister, introduced the <a href=\"https:\/\/www.canada.ca\/content\/dam\/hc-sc\/migration\/hc-sc\/hcs-sss\/alt_formats\/pdf\/pubs\/cha-ics\/2015-cha-lcs-ar-ra-eng.pdf\"><em><span style=\"color: #3366ff;\">Hospital Insurance and Diagnostic Services Act (1957)<\/span><\/em><\/a> that brought in public hospital insurance across the country. The funding arrangement was that the federal govt shared half the cost of provincial\/territorial hospital insurance plans. It faced strong opposition from pro-privatization forces wanting to bill patients directly (doctors, insurance companies, and large corporations).<\/p>\n<p>&nbsp;<\/p>\n<p><strong>1959:<\/strong> Ontario created a public provincial hospital insurance plan.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>1961:<\/strong><\/p>\n<ul>\n<li>All provinces and territories had implemented a <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/web.archive.org\/web\/20250223075953\/https:\/\/www.canada.ca\/en\/health-canada\/services\/health-care-system\/reports-publications\/health-care-system\/canada.html\">public hospital insurance plan<\/a><\/span>.<\/li>\n<li>\n<p style=\"font-weight: 400;\">In 1961, Saskatchewan passed legislation to expand their Medicare program to <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.cbc.ca\/history\/EPISCONTENTSE1EP15CH2PA4LE.html\">cover physician care outside of hospitals<\/a><\/span>(e.g. doctors\u2019 offices), prompting <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/policyalternatives.ca\/sites\/default\/files\/uploads\/publications\/National%20Office,%20BC%20Office\/2015\/04\/Cambie_Corp_Goes_to_Court.pdf\">a three-week strike<\/a><\/span> by doctors who wanted control and wanted to be able to extra-bill patients. The government held strong and brought in Public Medicare but with <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.cbc.ca\/history\/EPISCONTENTSE1EP15CH2PA4LE.html\">amendments<\/a><\/span> allowing doctors to opt out of the plan. By 1965, once it was in operation, the majority of doctors supported the plan.<\/p>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>1964:<\/strong> The federal Diefenbaker government\u2019s Royal Commission on Health Services led by Justice Emmett Hall called for national Public Medicare.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>1966: <\/strong>The <em>Medical Care Act (1966)<\/em> was passed so that doctors and medical care would be covered for all Canadians (not just in hospital). The federal govt would share half of the cost with the provinces\/territories.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>1972:<\/strong> <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.canada.ca\/en\/health-canada\/services\/health-care-system\/reports-publications\/health-care-system\/canada.html\">Each province and territory<\/a><\/span> had implemented Public Medicare plans for hospital and physician services.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>1984:<\/strong> In response to doctors extra-billing patients (charging patients extra user fees on top of OHIP\/the provincial public health plan), the <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.canada.ca\/en\/health-canada\/services\/health-care-system\/canada-health-care-system-medicare\/canada-health-act.html\">Canada Health Act<\/a><\/span> was passed, establishing five principles for our health care system and banning extra-billing and user fees.<\/p>\n<p style=\"font-weight: 400; padding-left: 40px;\"><span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.canada.ca\/content\/dam\/hc-sc\/migration\/hc-sc\/hcs-sss\/alt_formats\/pdf\/pubs\/cha-ics\/2015-cha-lcs-ar-ra-eng.pdf\">5 Principles of the Canada Health Act:<\/a><\/span><\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Portability: Health coverage must not end or stop when a person travels or moves between provinces\/territories in Canada.<\/li>\n<li>Accessibility: All insured individuals must have reasonable access to health care without barriers, such as financial or physical limitations.<\/li>\n<li>Universality: All Canadians must be covered by Public Medicare on equal terms and conditions.<\/li>\n<li>Comprehensiveness: All medically necessary hospital and physician services must be covered, even when the services are provided by another designed health professional.<\/li>\n<li>Public Administration<strong>: <\/strong>Provinces and territories must operate their health insurance plans on a non-profit basis. They must also be administered and operated by a public authority.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>1995:<\/strong> The federal Chr\u00e9tien government with Paul Martin (Jr.) as Finance Minister, brought in major cuts in the federal budget. The <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.healthcoalition.ca\/wp-content\/uploads\/2022\/05\/Fuller_HomeCare-What-we-need_2001.pdf\">Canada Health and Social Transfer (CHST)<\/a><\/span> combined funding for health, education and social services into one block fund and they cut federal health transfers by <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/Eroding-Public-Medicare.pdf\">40%<\/a><\/span>. These cuts, combined with provincial cuts, led to the expansion of private clinics.<\/p>\n<\/div>\n<p>&nbsp;<\/p>\n<p><strong>2001:<\/strong> The federal government created a National Commission (known as the Romanow Commission) on the future of Medicare, in part because of the crisis caused by the cuts.<\/p>\n<div>\n<p>&nbsp;<\/p>\n<p style=\"font-weight: 400;\"><strong>2002:<\/strong> The <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/Analysis-Nov-28-20021.pdf\">Romanow Report<\/a><\/span> called for the creation of the Canada Health Transfer (CHT) that would separate out health funding from other federal transfers and an escalator clause in the CHT to match inflation.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>2004:<\/strong><\/p>\n<ul>\n<li>\n<p style=\"font-weight: 400;\">Ontario\u2019s <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/Final-private-clinics-report-for-release.pdf\"><em>Commitment to the Future of Medicare Act<\/em><\/a><\/span> was passed to uphold the Canada Health Act. It bans extra-billing, user fees, queue jumping, and restricting access to insured services through block or annual fees. It sets out fines and jail terms for offenders who accept payment for an OHIP-covered service and requires the Ministry of Health to reimburse patients for unlawful extra-billing and user fees.<\/p>\n<\/li>\n<li>\n<p style=\"font-weight: 400;\">The <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.healthcoalition.ca\/wp-content\/uploads\/2017\/10\/Health-Accord-Report.pdf\">2004 Health Accord<\/a><\/span> introduced a <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.thestar.com\/opinion\/contributors\/ottawa-edges-away-from-public-medicare\/article_ba266ea6-3d3b-559a-964c-3b5b8c33436f.html\">6% escalator for the CHT<\/a><\/span> to offset the huge cuts made in the 1990s \u2013 although privatization had already gained a foothold. <span lang=\"EN-CA\">It also increased the federal share of health funding, not back to the historic 50-50 split with the provinces, but back to the 20% range (varying among provinces). <\/span><\/p>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>2006:<\/strong> The <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"http:\/\/www.healthcoalition.ca\/wp-content\/uploads\/2017\/10\/Health-Accord-Report.pdf\">election of the Harper government<\/a><\/span> halted progress as they sought to remove themselves from health care policy and <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.thestar.com\/opinion\/contributors\/ottawa-edges-away-from-public-medicare\/article_ba266ea6-3d3b-559a-964c-3b5b8c33436f.html\">refused to attend First Ministers&#8217; meetings.<\/a><\/span><\/p>\n<p>&nbsp;<\/p>\n<p style=\"font-weight: 400;\"><strong>2011:<\/strong> The Harper government announced cuts to the funding formula that governed CHT increases by <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"http:\/\/www.healthcoalition.ca\/wp-content\/uploads\/2017\/10\/Health-Accord-Report.pdf\">tying them to changes in GDP with a floor of 3%<\/a><\/span> starting in 2017, eliminating the 6% escalator agreed upon in the 2004 Accord. The impact was estimated by the provinces to be a reduction in <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.healthcoalition.ca\/wp-content\/uploads\/2017\/10\/Health-Accord-Report.pdf\">federal public health care funding by $36 billion over ten years<\/a><\/span> compared to what provinces would have received under the 6% escalator.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>2016:<\/strong> During the 2016-17 Health Accord negotiations, the Trudeau govt <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/Natalie-briefingnoteonCHTfundingproposal-3.pdf\">only minimally modified Harper\u2019s cut to the funding formula<\/a><\/span>. After the provinces and territories rejected their proposal, the Trudeau govt left the negotiating table. They then went province-by-province pushing through 10-year bilateral deals (that means different deals between the federal government and each province) to pressure all the provinces and territories into adopting the proposal.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>2022:<\/strong> Legislation to implement a national dental care insurance plan was passed. However, the federal govt contracted <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.cbc.ca\/news\/politics\/canadian-dental-care-plan-cdcp-seniors-coverage-begins-may-1-1.7189717\">a private insurance corporation (Sun Life)<\/a><\/span> to administer the plan.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>2024:<\/strong> The <span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.thetyee.ca\/News\/2024\/10\/28\/How-Federal-Pharmacare-Act-Affect-BC\/\">Pharmacare Act<\/a><\/span> was passed in October 2024 to create a national pharmaceutical insurance plan, starting with coverage for <a href=\"https:\/\/www.canada.ca\/en\/health-canada\/news\/2024\/05\/moving-forward-on-pharmacare-for-canadians.html\"><span style=\"color: #3366ff;\">contraceptives and diabetes medications<\/span><\/a>.<\/p>\n<\/div>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/History-of-Public-Medicare-fact-sheet-final.pdf\"><span style=\"color: #3366ff;\">Click here for printable version of fact sheet<\/span><\/a><\/span><\/p>\n<p><span style=\"color: #3366ff;\"><a style=\"color: #3366ff;\" href=\"https:\/\/www.ontariohealthcoalition.ca\/wp-content\/uploads\/History-of-Public-Medicare-briefing-note-final.pdf\">Click here for printable version of the more in-depth briefing note<\/a><\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>(November 27, 2024) &nbsp; &nbsp; &nbsp; Fact Sheet The History of Public Medicare &nbsp; What is Public Medicare?
