As of November 1, 2020- The Ontario Health Coalition has compiled this list from trusted media sources, hospital and public health websites, and by calling hospitals and assessment centres directly to confirm information. Please note things are changing rapidly. Different assessment centres have different criteria for access. Some do testing on site, some do not. This information is correct as of today. Please call or visit the websites of the hospital/assessment centres or public health units indicated below to check current status. As tests become more available they may be able to test more people. New assessment centres are being added daily.

Northern Ontario:

Atikokan: An assessment centre has been opened at the Atikokan Family Health Team at 101 Zuke Road.  If you have symptoms and need to be tested, please call 807-597-8781 ext0 they will make an appointment with you.  This location is not open for walk-ins. Call lines are open Monday to Friday, 8:30 a.m. to 4:30 p.m.

Attawapiskat: An assessment centre has been opened at the Weeneebayko Area Health Authority at 972 Riverside East. If you need to be tested, please call 705-336-2294 ext. 326 and they will make an appointment with you. This location is open Monday to Friday from 8:30 a.m. to 4:30 p.m. and is closed for lunch from 12:00 p.m. to 1:00 p.m.

Blind River: An assessment centre has been opened by the North Shore Health Network in Blind River, located in close proximity to the Emergency Department at 525 Causley Street. Community members with potential COVID-19 symptoms who feel they need to be assessed for COVID-19 should fill out this online form or call 705-356-2265 ext. 2661. Once you submit the form, you can expect to be contacted by the Centre through a telephone conversation within 48 hours. On the call, patients will be screened for travel and contact history and symptoms, and guided to self-monitor, self-isolate at home, come to the Assessment Centre or go to the Emergency Department. Not every patient will be tested. Not everyone who attends the Centre will be swabbed. Hours of operation are Monday – Sunday: 8:00 a.m. to 6:00 p.m.

Chapleau: An assessment centre has been opened in the portable office beside the Chapleau and District Family Health Team building, 2 Broomhead Rd. The Assessment Centre is open by appointment only from Monday to Thursday, 12:00 p.m. to 4:00 p.m. and Friday, 9:00 a.m. to 1:00 pm. Drive through is available for those who qualify. Walk-ins are not permitted due to health and safety reasons. If you need to be tested, please contact the Chapleau COVID-19 Assessment Centre at 705-864-2568 to book an appointment to be tested as soon as possible. Phone lines are open from Monday to Friday, 8:30 a.m. to 4:30 p.m.

Dryden: There is an assessment centre located at the Dingwall Medical Clinic Building on 40 Goodall Street in Dryden. The hours of operation are Monday to Thursday, 8:30 a.m. to 4:30 p.m and Fridays from 8:30 a.m. to 1:00 p.m. The assessment centre is by appointment only. Residents displaying COVID-19 symptoms are asked to call the Northwestern Health Unit COVID-19 hotline at 1-866-468-2240 so they may determine if you require an appointment at the DRHC Assessment Centre. If you require an appointment, the Northwestern Health Unit will contact the DRHC Assessment Centre to identify your need for an appointment. The DRHC Assessment Centre will then contact you to schedule the appointment. Anyone over the age of 6 months can be tested.

Elliot Lake: An assessment centre has opened at Elliot Lake Family Health Team, located at 31 Nova Scotia Walk, Elliot Lake. The hours of operation are Monday to Friday between the hours of 8:30 am and 4:30 pm. Patients exhibiting symptoms of COVID-19 should self-isolate for fourteen days. Patients are asked not to visit the Emergency Department unless it is a medical emergency. Patients should call the Elliot Lake Family Health Team at 705-461-8882 and press 1 if they are concerned that they could have been exposed to COVID-19. Patient inquiries will be screened and telephone interviews will be conducted to determine if a patient should be tested. Under physician orders, a Registered Nurse will be deployed to conduct the COVID-19 test at the patient’s home.

Espanola: A mobile COVID-19 assessment centre has opened for the residents of Espanola. The response team can be accessed by appointment only. Referrals can be made by any of the following services: Telehealth Ontario, Public Health Unit Sudbury & Districts, Sagamok Wellness Centre, Birch Island Health Centre, or by a family physician’s office. Individuals meeting the criteria below are to call the ERHHC Assessment Centre at 705-869-1420 ext. 4500 between the hours of 8am and 4pm, Monday to Friday instead of presenting to the Emergency Department. The response team will run from 8:00 am to 8:00 pm seven days a week.

Fort Albany: An assessment centre is open at 5 Airport road in Fort Albany. The hours of operation are from 8:30 a.m. to 4:30 p.m. Monday to Thursday. Call 705-336-2294 ext. 326 to make an appointment.

Fort Frances: An assessment centre is open in Fort Frances at the Community Counselling Building, 206 Victoria Avenue. The centre is operating seven days a week between the hours of 8:00 am and 4:00 pm (Saturday-Sunday as needed according to Public Health Ontario). Use the Victoria Street entrance and please note on-site directions will be given. The assessment centre sees patients by appointment only. Walk-in appointments are prohibited. Contact the assessment centre at 1-807-274-3261 ext. 4913 to book an appointment.

Geraldton: An assessment centre operated by the Geraldton District Hospital is now open at the Northern Horizon Health Center, located at 510 Hogarth Ave. W adjacent to the hospital. The centre will operate from Monday to Friday, from 8:00 am to 4:00pm, according to Geraldton District Hospital. During off hours, please report to the Emergency Department to be assessed. All patients requiring medical assistance are asked to please report to the assessment centre where you will be screened. The Physician will determine based on pre-set criteria whether or not you require testing for COVID-19. You can book an appointment at 804-854-0454.

Hearst: An assessment centre is open in Hearst at the Nord-Aski Family Health Team building on 1403 Edward Street. Hours of operation are Monday to Friday: 8:00 a.m. to 4:00 p.m. The assessment centre does not take walk-ins. If you think you are experiencing symptoms of COVID-19 or you have been in close contact with someone with COVID-19 and need a referral to the Assessment Centre, you may call the assessment centre at 705-362-5544 to book your appointment directly.

Hornepayne: An assessment centre is open in Hornepayne at the Hornepayne Community Hospital on 278 Front Street. Hours: 9:30 to 11:30 am and 1:00 to 2:00 pm, Monday to Friday. The assessment centre does not take walk-ins. If you think you are experiencing symptoms of COVID-19 or you have been in close contact with someone with COVID-19 and need a referral to the Assessment Centre: 1) Complete the online self-assessment tool and call the Porcupine Health Unit, your primary care provider, or Telehealth if recommended, 2) A public health nurse, your primary care provider or the nurse at Telehealth will review your results with you and ask other questions, 3) If further assessment is required, you will be referred to the Assessment Centre, 4) Someone will call you to schedule the appointment. COVID tests at the assessment centre are usually done in the mornings until 11:00 a.m. For more information contact the Porcupine Health Unit COVID-19 hotline at 1-807-868-2442, they can be reached during the week from 9:00 am to 5:00 pm.

Iroquois Falls: An assessment centre is open at the Anson General Hospital, located at 58 Anson Drive. Hours of operation are Tuesdays and Thursdays: 9 a.m. to 1 p.m. The assessment centre does not take walk-ins. If you think you are experiencing symptoms of COVID-19 or you have been in close contact with someone with COVID-19 and need a referral to the Assessment Centre, you may call the assessment centre at 1-888-962-8718 to book your appointment directly.

Kapuskasing: An assessment centre is open in Kapuskasing at Sensenbrenner Hospital on 7 Aurora Avenue. Hours of operation are Tuesdays, Thursdays, and Fridays: 8:30 a.m. to 11:30 a.m. The assessment centre does not take walk-ins. If you think you are experiencing symptoms of COVID-19 or you have been in close contact with someone with COVID-19 and need a referral to the Assessment Centre: 1) Complete the online self-assessment tool and call the Porcupine Health Unit, your primary care provider, or Telehealth if recommended, 2) A public health nurse, your primary care provider or the nurse at Telehealth will review your results with you and ask other questions, 3) If further assessment is required, you will be referred to the Assessment Centre, 4) Someone will call you to schedule the appointment. For more information contact the Porcupine Health Unit COVID-19 hotline at 705-267-1181 or 1-800-461-1818, they can be reached during the week from 9:00 am to 5:00 pm. You may call 705-367-4613 to book your appointment directly.

Kenora: An assessment centre has been opened at the Lake of the Woods District Hospital at 21 Sylvan Street. Hours of operation are Monday – Friday: 8:15 a.m. – 3:45 p.m., according to the Covid-19 testing locations page on Ontario.ca. The assessment centre sees patients by appointment only. Walk-ins are prohibited. Can drive-through. To schedule an appointment, call the assessment centre at 1-807-​468-9861 ext. 2483. Please note, the assessment centre is slated to be moved to the former land ambulance base next to Paterson Medical Clinic.

Killaloe: An assessment centre is open at 34 Cameron St in Killaloe ON. The hours of operation are from 2:00 p.m. to 3:30 p.m. Thursday. Call 1-844-727-6404 to make an appointment.

Kirkland Lake: A drive-thru assessment centre is open at Kirkland Lake Hospital, located at 145 Government Road East, Kirkland Lake ON, P2N 3P4. The assessment centre sees patients by appointment only. Please call to check if newborns or young children can be tested. Hours: 8:10 a.m. to 1 p.m., Monday to Friday. Individuals may contact the assessment centre to book an appointment at 705-568-2127.

Manitoulin: Assessment centres have opened at both the Little Current and Mindemoya hospital sites to test possible cases of the virus away from the main hospital. COVID-19 testing is by appointment only. Mindemoya is located at 20120B Hwy 551 and Little Current is located at 11 Meredith Street East. The assessment centres only test individuals older than two years of age. Anyone wishing to be tested must have a fever above 38 degrees Celsius and a new or worsening cough. They must have, in the 14 days before experiencing symptoms, been outside of Canada, had close contact with a confirmed or probable COVID-19 case or had close contact with someone who had COVID-19 symptoms and was themselves outside of Canada. Those who match the above criteria must call MHC at 705-368-2300 for Little Current or 705-377-5311 for Mindemoya and follow the phone prompts to reach the COVID-19 assessment line. That phone number will be monitored from 9:00 am to 5:00 pm every day. For the people whose symptoms warrant an in-person assessment, they will be given a time slot to visit one of the two sites which are open from 1:00 pm to 5:00 pm from Monday to Sunday.

Manitouwadge: An assessment centre has been opened at the Sante Manitouwadge Health at 1 Health Care Crescent. If you need to be tested, please call 807-826-3251 ext. 221 and they will make an appointment with you. This location is open Monday to Sunday from 8 a.m. to 7 p.m.

Matheson: An assessment centre is open at 507 8th Avenue in Matheson ON. The hours of operation are from 9:00 a.m. to 3:00 p.m. on Wednesday.  Call 1-888-962-8718 to make an appointment.

Mattawa: An assessment centre has been opened at Mattawa Hospital at 217 Turcotte Park Rd. If you need to be tested, please call 705-744-5511 ext. 0 and they will make an appointment with you. This location is open Monday, Wednesday, and Friday from 8 a.m. to 12 p.m.

Marathon: An assessment centre is open at the office of the Marathon Family Health Team, at 26 Peninsula Road. Hours: 8:30 a.m. to 4:30 p.m., Monday to Friday. Drive-through is available. The assessment centre sees patients by appointment only. If you are concerned that you may have COVID-19, please call the Marathon Family Health Team at 807-229-3243, or the local Public Health Unit at 807-229-1820. They will ask you questions over the telephone to determine if you should be tested. If you require a test, an appointment with the assessment centre will be made for you and they will provide you with further instructions.

Moose Factory: An assessment centre has been opened at the Weeneebayko General Hospital at 19 Hospital Drive. If you need to be tested, please call 1-877-658-9242 or 705-658-4220 between 8:30 a.m. and 5 p.m. (Monday – Friday) and they will make an appointment with you. This location is open Monday to Thursday from 1 p.m. to 4 p.m. They test anyone over 2 years of age.

Moosonee: An assessment centre has been opened at the Weeneebayko Area Health Authority at 5 Percy’s Way. If you need to be tested, please call 705-336-2294 ext. 326 and they will make an appointment with you. Please call to check if newborns or young children can be tested. This location is open Monday to Friday from 8 a.m. to 4 p.m. (8:30 a.m. to 12 p.m. and 1 p.m. to 4:30 p.m., Monday to Friday).

Nipigon: An assessment centre has been opened at Nipigon District Memorial Hospital, at 125 Hogan Road. If you need to be tested, please call 807-887-5252 and they will make an appointment with you. You can call to check if they test newborns and young children. This location is open Monday to Friday from 9 a.m. to 3 p.m.

Nipissing: An assessment centre has been opened by the West Nipissing General Hospital in the Marcel Noel Hall at 219 O’Hara Street in Sturgeon Falls. For health and safety reasons, the COVID-19 Assessment Centre is by appointment only, and walk-ins are prohibited. To make an appointment or ensure that newborns or young children are tested, please call the Assessment Centre at (705) 580-2186. Hours of operation are Monday, Tuesday, Thursday, and Friday: 12 p.m. (noon) to 4 p.m.

North Bay: An assessment centre is open at the North Bay Regional Health Centre (NBRHC), located at 50 College Dr. The hours of operation are Monday – Sunday: 9 a.m. to 5 p.m. The assessment centre has an entrance on the west end of the building and has been sealed off from the rest of the hospital. To book an appointment, call 705-474-8600 ext. 4110. A drive-thru option is also available at the west entrance of NBRHC.

Ohsweken: An assessment centre is open at 1738 Fourth Line Rd in Ohsweken ON. The hours of operation are from 8:00 a.m. to 8:00 p.m. from Monday to Sunday.  This location is only available to Six Nation Members and Providers only and they have a drive through service available.

Peawanuck: An assessment centre has been opened at the Peawanuck Nursing Station by the Weeneebayko Health Authority. If you need to be tested, please call 705-336-2294 ext. 326 and they will make an appointment with you. You can call to check if they test newborns and young children. This location is open Monday to Friday from 8 a.m. to 4 p.m. (8:30 a.m. to 12 p.m. and 1 p.m. to 4 p.m., Monday to Friday)

Rainy River: An assessment centre is open in Rainy River at the Rainy River Health Centre, 115 Fourth Street. The centre is operating seven days a week between the hours of 1:00 pm and 3:00 pm. The entrance to the assessment centre is off Mill Avenue, near the back of Rainy River Health Centre. On-site directions will be given. The assessment centre sees patients by appointment only. Walk-in appointments are prohibited. To schedule an appointment, call the assessment centre at 1-807-274-3261 ext. 6500.

Red Lake: An assessment centre is open in Red Lake at the Red Lake Margaret Cochenour Memorial Hospital, located at 51G Highway 105. The hours of operation are Sunday to Thursday: 9 a.m. to 2 p.m. Access to this centre is by appointment only.  Walk-in appointments are prohibited. Please contact the Assessment Centre at 807-727-7035 between the hours of 9:00 am and 12:00 pm (noon), Monday through Friday to schedule an appointment, if necessary.

Sault Ste. Marie: Assessments and testing are being done at the former Kal Tire location at 165 Drive In Road, near the Sault Area Hospital. Patients must call first and be screened by phone. Walk-ins are prohibited. You must have symptoms (fever over 38 degrees Celsius) and/or cough (new or worsening) and in the last 14 days prior to the onset of symptoms have travelled outside of Canada, or had close interactions with a confirmed or probable case of COVID-19, or had close interactions with someone who has symptoms of COVID-19 who recently travelled outside Canada or to an impacted area within Canada. People in the Algoma region experiencing symptoms are encouraged to call the COVID-19 Assessment Centre at 705-759-3434 ext. 7152 between the hours of 7:30 a.m. – 7:30 p.m., 7 days per week. People will be assessed by phone and scheduled to go in if they meet the criteria. Testing is conducted for anyone older than 2 years. Individuals with general questions should not call Sault Area Hospital. Instead, please contact Algoma Public Health at (705) 759-5404 (toll-free 1-866-892-0172 ext. 5404) or visit their website.

Sioux Lookout: There is an assessment centre located at Sioux Lookout Meno Ya Win Health Centre on 1 Meno Ya Win Way in Sioux Lookout. Access to this centre is by appointment only. The hours of operation are Monday to Friday from 8:00 a.m. to 4:00 p.m. If you have a fever or cough and have either returned from international travel in the last 14 days or been in contact with someone who has COVID-19, contact the Assessment Centre at 1(866) 468-2240.

Sudbury: Assessments and testing are being done a block from the hospital at Health Sciences North Research Institute at 56 Walford Rd in Sudbury. This screening is by appointment only. Walk-ins are prohibited. For an appointment at the assessment centre the following criteria must be met: Fever (over 38 degrees Celsius) and/or cough (new or worsening) and in the last 14 days prior to the onset of symptoms have travelled outside of Canada, or had close interactions with a confirmed or probable case of COVID-19, or had close interactions with someone who has symptoms of COVID-19 who recently travelled outside Canada or to an impacted area within Canada. For patients who may have difficulty going in for an assessment, arrangements will be made for an assessment at home from an employee of the Greater Sudbury Community Paramedicine Program. An individual who meets the criteria above must call the assessment centre at 705-671-7373 between 9 a.m. and 5 p.m. rather than going to the hospital’s emergency department. The assessment centre can test anyone over 2 years of age. Basic intake information will be taken and a clinician will call them back for the next steps. Anyone with general questions should call Public Health Sudbury & Districts at 705-522-9200 or toll-free 1-866-522-9200 and not the hospital. As of April 10, a drive-thru option has been added to the Sudbury assessment centre. Drive-thru testing is also being done by appointment only.

A second assessment centre has been opened at NEOMO Medical on 885 Prete Street. Drive-through is available. The hours of operation are 8 a.m. to 4 p.m., seven days a week. Testing is by appointment only. Patients will be screened by a nurse over the telephone after calling Health Sciences North at 705-671-7373 and will be referred to an assessment centre if necessary. You can also book an appointment online.

A drive-through assessment centre has also been opened at Primacy Medical Centre on 1485 Lasalle Boulevard. The centre is located in the Superstore parking lot, next to the pharmacy. The hours of operation are 11 a.m. to 7 p.m., seven days a week. Testing is by appointment only.  The centre can test anyone older than 6 months. Patients will be screened by a nurse over the telephone after calling Health Sciences North at 705-671-7373 and will be referred to an assessment centre if necessary.

Temagami: An assessment centre is open at 17 O’Connor Dr in Temagami ON. The hours of operation are from 8:30 a.m. to 4:30 p.m. on Monday to Friday. Please call 705-569-3244 to book an appointment.

Temiskaming: An assessment centre is open at Temiskaming Hospital, located at 421 Shepherdson Rd. The drive-thru assessment centre is located at the North entrance of the hospital. The hours of operation are Monday to Friday: 9 a.m. to 12 p.m. (noon) and 1 p.m. to 4 p.m. and Sunday: 10 a.m. to 12 p.m. (noon). The assessment centre testing is by appointment only. To book an appointment, contact the assessment centre at 705-648-1844. The centre can test anyone over 2 years old.

Terrace Bay: An assessment centre is open at the McCausland Hospital, located at 20B Cartier Road. Hours of operation are Monday to Friday from 8:30 a.m. to 4:30 p.m. The assessment centre sees patients by appointment only. If you are concerned that you may have COVID-19, please call your family doctor, the local Public Health Unit at 807-825-7770, or the centre at 807-825-3235. They will ask you questions over the telephone to determine if you should be tested. If you require a test, an appointment with the assessment centre will be made for you and they will provide you with further instruction.

Thessalon: An assessment centre has been opened by the North Shore Health Network in Thessalon, located in close proximity to the Emergency Department at 135 Dawson Street. The hours of operation are 7 days a week from 8 a.m. to 6 p.m. Community members with potential COVID-19 symptoms who feel they need to be assessed for COVID-19 should fill out this online form. Once you submit the form, you can expect to be contacted by the Centre through a telephone conversation within 48 hours. On the call, patients will be screened for travel and contact history and symptoms, and guided to self-monitor, self-isolate at home, come to the Assessment Centre or go to the Emergency Department. Not every patient will be tested. The centre can test anyone older than 2 years. Not everyone who attends the Centre will be swabbed. You can call 705-356-2265 ext. 2662 or toll free: 1-888-425-0321

Thunder Bay: The assessment centre is located at Superior North EMS Community Paramedicine Program at Superior North EMS, 105 Junot Avenue South. The hours of operations are Monday to Friday from 8:00 am to 4:00 pm and Saturday – Sunday 9 a.m. to 1 p.m. Appointments are preferred. The centre also has drive-through options available. Please call to check if the centre tests all ages. To be assessed and scheduled for testing contact the Thunder Bay Assessment Centre located at the Thunder Bay Regional Health Sciences Centre (TBRHSC). You can call (807) 935-8100 or Toll-free 1 (888) 294-6630 from Monday to Friday. You can also book online at http://tbh.net/bookonline.

Thunder Bay Regional Health Science Centre: 980 Oliver Road, Thunder Bay, ON P78 6V4, Monday-Sunday: 8:00 a.m. to 4:00 p.m.

Timmins: An assessment centre has been set up in Timmins, located at 651 Ross Avenue East (Dr Verbeek’s office, across from the Timmins and District Hospital). The centre is open Monday to Friday by appointment only from 1:15p.m. to 4:00 p.m. and accepts walk-ins from Monday to Sunday 8:30 am to 12pm. A referral is required from a family doctor, Public Health or Telehealth. The local health unit can be reached at 705-267-1181 or 1-800-461-1818. The number for Telehealth Ontario is 1-866-797-0000. You may also book your appointment directly by calling 705-267-0224. You can call to check if they test newborns and young children.

Wawa: As of September 16th, the assessment centre has been moved to a new location: Lady Dunn Health Centre, located at 17 Government Road. Hours: Monday to Friday 8:30 am to 4:30 pm. The assessment centre sees patients by appointment only. The centre tests individuals aged 2 and over. To book an appointment, call the assessment centre at 705-914-1114.

 

Southwestern Ontario:

Brantford: An assessment centre is open at 79 Market St S in Brantford. The hours of operation are from 8:00a.m to 4:00p.m. and they are open from Monday to Friday. Call 519-751-5818, email covid.assessment@bchsys.org, or book online https://www.bchsys.org/en/covid-19-online-scheduling.aspx to make an appointment.

Cambridge: An assessment centre is open at 10 Grand Avenue West in Cambridge. The hours of operation are from 8:30 a.m. to 8:30 p.m. Monday to Friday and 8:30 a.m. to 4:30 p.m. on Saturday and Sunday. Call 226-895-1050 to make an appointment.

Chatham: The Chatham-Kent Health Alliance (CKHA) has a COVID-19 Assessment Centre at 47 Emma Street in Chatham in partnership with the Chatham-Kent Public Health Unit. The hours of operation are Monday to Sunday from 9 a.m. to 5 p.m. The last patient of the day will be registered at 4:20 p.m. It is recommended that you book an appointment to be seen in a timely manner. CKHA offers an online booking tool to schedule an appointment at the COVID-19 Assessment Centre.  To access the booking tool, please visit the website www.assessmentbooking.ca on any internet-enabled device and complete the online form to schedule an appointment. Before booking an appointment, please make sure you have a valid “green” Ontario health card.  Please provide an email address while filling out the online form to confirm your appointment date and time.

Patients who have a “red and white” Ontario health card, or those who do not have access to technology, can book an appointment by calling CKHA’s Registration at 519.352.6400 ext. 6548 between the hours of 9 a.m. to 5 p.m., Monday to Friday.  After hours and on weekends, patients can call CKHA’s Switchboard at 519.352.6400 ext. 6584.

Guelph: An assessment centre is open at 400 Southgate Drive. The centre is open seven days a week from 8:45 a.m. to 4:15p.m. Individuals aged 3 months and older can be tested. The centre is advising residents not to visit the centre if they do not have symptoms or if symptoms are mild. If symptoms are moderate with fever, cough or shortness of breath, the centre is advising residents to call their family doctor to arrange an over-the-phone assessment. The family doctor may decide that it is best for you to go to the assessment centre. Alternatively, you can call Wellington-Dufferin-Guelph Public Health at 1-800-265-7293 ext. 7006 to conduct an over the phone assessment with a public health nurse. If deemed necessary, they will schedule you an appointment at the assessment centre.

Hagersville: An assessment centre is open in Hagersville at West Haldimand General Hospital located at 75 Parkview Road. Harriston: An assessment centre is open at 83 Arthur St W in Harriston ON. The hours of operation are Tuesday, Thursday and Friday from 9a.m to 5p.m and are closed for lunch from 12:30p.m-1:30p.m. Call 519-843-8160 or visit https://www.whcacovid.com/harriston-assessment-centre to make an appointment.

Kitchener: A drive-thru assessment centre is located at 15 Charles St. W. The centre is open 7 days a week: 7:00 a.m. to 6:45 p.m. Appointments can be booked online at https://covidtestregistration.grhosp.on.ca/Registration/RegistrationHours. The centre is operated by Grand River Hospital and can test anyone over 2 years old.

Leamington: Erie Shores Health Care has a COVID-19 Assessment Centre within the hospital located on 194 Talbot St W, that opened on Thursday April 16th. The hours of operation are Monday to Sunday from 12:00p.m to 5:00p.m. The last assessments will be at 3:30pm daily. The COVID-19 Assessment Centre will be on a walk-in basis with no referral or pre-screen by primary care, Public Health, or Telehealth required. Anyone over 1 year old can be tested. The COVID-19 assessment unit can be reached at 519-326-2373 ext. 4263 in case there are any questions during business hours.

London:  An assessment centre in London is open at the Carling Heights Optimist Community Centre at 656 Elizabeth St. Hours are from 9:00 am to 5:00 pm, from Monday to Friday. The Health Unit recommends anyone concerned first consider a self-assessment, under guidelines provided by the province’s self-assessment tool found here (https://covid-19.ontario.ca/self-assessment/#q0). Anyone looking for information is advised to call the Middlesex London Health Unit at 519-663-5317.

Western University is providing COVID-19 testing on campus for students and staff. Testing is available at the mobile testing trailer, located in the Social Science Centre parking lot.

Hours: Monday to Friday from 11:00 a.m. to 6:00 p.m. and Saturdays from 11:00 a.m. to 3:00 p.m.

Sarnia-Lambton: The assessment centres can be found in the following locations and calling ahead to confirm testing is preferred:

The Rural Lambton Covid-19 Assessment Centre is a drive-thru assessment centre located at 596 Broadway street, Wyoming and can be reached at 519-882-2500. Spanish and Arabic language is available. The hours of operations are Monday and Wednesday: 12:30pm to 7:00pm (closed 4:30 to 5:30 for supper), Friday: 9:00am to 4:00pm (closed 12:00 to 1:00 for lunch), and Saturday: 9:00am to 12:00 pm. The Grand Bend Area Community Assessment Centre is located at 69 Main Street East, Grand Bend and can be reached at 519-238-2362. The centre is open Monday to Friday from 1:30 pm to 4 pm (excluding holidays).

Indigenous Health Centre:

Kettle & Stony Point First Nation is operating through a strong partnership developed with officials at Bluewater Health and Lambton Public Health. The centre is located at 6247 Indian Lane and can be reached by 519-786-5647. The tests will be administered by appointment only. The hours of operations are Tuesdays from 12:00 p.m. (noon) to 4:00 p.m., Wednesday 3:00 p.m. to 7 p.m. and Thursday from 12:00 p.m. (noon) to 7:00 p.m.

Six Nations of the Grand River: Six Nations of the Grand River currently has a COVID-19 Call-in Centre which can be reached at 1-855-977-7737 or 226-446-9909. Please call if you would like more information.

St. Thomas: An assessment centre is open at the St Thomas Elgin General Hospital, located at 189 Elm Street. The centre is located at the hospital and is accessed through a dedicated entrance on Hepburn Avenue. The hours of operation are Monday to Sunday from 8 a.m. to 6 p.m. The centre sees patients by appointment only. To schedule an appointment, call 519-631-2030 ext. 6210 or email: assessmentcentre@stegh.on.ca. Individuals are asked to leave the following information on the voicemail, and an employee will call you back with a scheduled appointment time: Name, Phone Number, Health Card Number (if applicable), Primary Care Physician, and list any symptoms you are experiencing. Please advise if you work in health care/long-term care or if you have an upcoming surgery. Patients without symptoms of COVID-19 will be scheduled 3-4 days out.

Stratford: An assessment centre has been opened at Rotary Complex-Hall D At 353 McCarthy Road, Stratford. Appointments only. It is a drive-through, however people without a vehicle will also be accommodated. This location is open Monday to Friday 9:15am to 4:30pm and can test anyone over 2 years of age.

Waterloo: An assessment centre is open at the St. Mary’s General Hospital Cardiac Rehabilitation Centre located at 50 Bathurst Drive, Unit 1, Waterloo. The hours of operations are Monday to Sunday from 9 am to 5 pm. Self-referrals are accepted by calling 519-885-9517.

Windsor: An assessment centre has been opened at the Windsor Regional Hospital at the former outpatient clinic entrance (off Goyeau St.). It is a segregated area next to the Ouellette campus emergency department on 1030 Ouellette Avenue. The hours of operation are from 7:00 a.m. to 3:00 p.m. seven days a week. Please note, there will be no access into the main hospital from this area or access into the COVID-19 Assessment Centre from the main hospital. All-access will be from the entrance off Goyeau Street. Online appointments can be booked at www.wrh.on.ca/OnlineBooking Before going to the assessment centre, people who are concerned about potential exposure are asked first to call their primary care provider (family doctor/nurse practitioner or equivalent), or the Windsor-Essex County Health Unit 519-258-2146 ext. 1420 or Telehealth 1-866-797-0000. The hospital asks that you check their dedicated COVID-19 webpage for updates as this information is subject to change at any time.

Woodstock: There is an assessment centre located at Woodstock Hospital on 310 Juliana Drive. There is a trailer on hospital property serving as the assessment centre. The hours of operation are Monday to Sunday from 10 a.m. to 6:00 p.m. At this time, this is not a walk-in or drive up assessment centre. Appointments only. Patients must make an appointment to this assessment centre by calling 1-855-905-6148 between the hours of 8:00 am to 6:00 pm Mon-Fri or Sat-Sun 8:00 am to 4:00 pm. Hamilton/Niagara:

Dunnville: An assessment centre is open at 400 Board St in Dunnville. The hours of operation are from 9:00 a.m. to 11:00 a.m. on Wednesday, there is also a drive-through available.  Call 905-774-7431 ext. 1293 to make an appointment.

Hamilton:  An assessment centre is in Hamilton Health Sciences West End Clinic at 690 Main St. W. The West End Clinic is open from 9:00 a.m. to 5:00 p.m. daily. Hamilton test centres will only see people by appointment who have a referral from their doctor or public health. You can contact Hamilton Public Health Services at 905-974-9848 between 8:30 a.m. – 4:30 p.m. to get an appointment. Voice messages can be left during this time and will be returned. Voice messages will not be accepted outside of business hours. A potential third centre on standby is the Stonechurch Family Health Centre. A second assessment centre is open in St. Joe’s Mountain at 135 Fennell Avenue West in Hamilton. The hours of operation are from Monday to Friday 8:00 to 10:30pm, Saturday and Sunday 8:00 am to 4:00pm.

Niagara: An assessment centre is open at Niagara Health Greater Niagara General Site at 5546 Portage Rd in Niagara Falls. Toronto:

Assessment centres are open at the following sites. They have different rules and criteria.

Crescent Town Health Centre – Temporary Site: An assessment centre is open at the Crescent Town Health Centre at 4 The Market Place in East York. Hours of operation are Tuesdays, Thursdays, and Saturdays from: 1 p.m. to 6:30 p.m. Please call 647-477-1640 to make an appointment. Walk-ins available for testing, vaccines and take-home test kit pick-up and drop-off. Case management services are also offered at this location. Registration ends at 6:15 p.m. – please arrive before this time to ensure you receive a test/vaccine.

Humber River Regional Hospital: An assessment centre is open at 2111 Finch Avenue West. Hours: 7 days a week Monday – Friday: 11:00 a.m. to 7 p.m. and Saturday – Sunday: 8:00 a.m. to 7:00 p.m. Anyone over 6 months of age can be tested. Use the west entrance for assessments. People are asked to do a self-assessment before showing up at the centre. For more information call the centre at 416-747-6740. The hospital is advising patients to call Telehealth Ontario first to be screened at 1-866-797-0000 or use the self-assessment tool prior to going to the assessment centre.

Na-Me-Res Auduzhe Mino Nesewinong (Place of Healthy Breathing): An assessment centre is open at 22 Vaughan Rd in Toronto. The hours of operation are:

  • Monday: 1 p.m. to 5 p.m.
  • Tuesday: 4 p.m. to 8 p.m.
  • Friday: 10 a.m. to 2 p.m.
  • Saturday: 11 a.m. to 3 p.m

Michael Garron Hospital (East Toronto): An assessment centre is open at the hospital at 825 Coxwell Avenue, outpatient clinic location in D-wing, 1st floor; accessed from Mortimer Avenue. No walk-ins. By appointment only. Hours: Tuesday through Sunday: 7 am to midnight and Monday: 8 am to 8 pm. The COVID-19 Assessment Centre provides screening, assessment and testing for individuals in East Toronto who are experiencing possible symptoms of COVID-19. Community members with potential COVID-19 symptoms (e.g. fever, new or worsening cough, sore throat, headache, muscle aches, fatigue, runny nose and joint aches) who feel they need to be assessed for COVID-19 should phone the Assessment Centre. On the call, patients will be screened for travel and contact history and symptoms, and guided to self-monitor, self-isolate at home, come to the Assessment Centre, or go to the Emergency Department. The Assessment Centre is not open for walk-in visits. Please call ahead to book an appointment: 416-469-6858. Patients who need to go to the Assessment Centre will be offered a booked appointment. The hospital’s 24-hour emergency department has a dedicated zone for assessment also.

A temporary assessment centre has been opened by the Michael Garron Hospital at 45 Overlea Blvd at Thorncliffe Park. The hours of operation are 7 days a week from 1:00 p.m. to 7:00 p.m. Please call 647-477-1640 to make an appointment. Walk-ins available for testing, vaccines and take-home test kit pick-up and drop-off. Registration ends at 6:45 p.m. – please arrive before this time to ensure you receive a test/vaccine.

Mount Sinai Hospital (downtown): An assessment centre is open at Mount Sinai hospital, located at 600 University Avenue on the ground floor. The entrance to the assessment centre at Mount Sinai Hospital is located just north of the main entrance at 600 University Avenue. Hours: 8:00 am to 4:00 pm, on Monday, Tuesday, Wednesday and Friday and 8:00 am to noon on Thursday. Before visiting a COVID-19 assessment centre, please use this online self-assessment tool (https://covid19toronto.ca/).

North York General Hospital (Leslie St. just north of the 401): An assessment centre located on the 3rd floor east room 300 and is open Monday to Friday from 9:00 a.m. to 5:00 p.m. Call: 416-635-2509 to book an appointment. The hospital is encouraging people to do a pre-assessment by calling Telehealth Ontario (1-866-797-0000) or by using the Toronto COVID-19 Self-Assessment Tool at https://covid19toronto.ca/. There are no further requirements at this time. For more information call the main line at 416-756-6000.

Scarborough: A mobile assessment centre is open visiting various locations in Scarborough. There are currently no upcoming dates and locations for the Pop-up Assessment Centres. For COVID-19 testing. Patients are advised to visit the Assessment Centres at Birchmount and Centenary hospitals.

Scarborough Hospital – Birchmount Road site: An assessment centre is located in front of the hospital, located at 3030 Birchmount Road. At this time, patients will not be tested unless they are demonstrating serious symptoms. The hours of operation are seven days a week from 8:00 a.m. to 6:30 p.m. The hospital is encouraging people to do self-assessment first online by visiting www.shn.ca/covid-19. Scarborough Health Network – Centenary Hospital: An assessment centre is open in Scarborough at the Centenary hospital, located at 2867 Ellesmere Rd. The centre is open seven days a week from 8:00 a.m. to 6:30 p.m. The centre is located outside of the hospital building, on the East side heading toward the Emergency department, and is accessible from Neilson Rd. Please follow the directions on-site. Visit the centre only if you have symptoms of a respiratory tract infection or fever, including cough, sore throat, headache, muscle aches, fatigue, runny nose, or joint aches, AND if you have been instructed by Public Health to visit the centre, or if you work in a vulnerable or at risk setting. These settings include, school or childcare centres, retirement homes, homeless shelters, prisons, or any health care setting such as hospitals, long-term care facilities, complex continuing care, rehabilitation centres, and dialysis centres, or other settings with vulnerable populations. Sunnybrook Health Sciences Centre (Blythwood Rd. North of Eglinton Ave. E. and Bayview Ave.): An assessment centre is now open at the hospital. People who are going to the Assessment Centre must enter through the U-Wing entrance. Do not enter Sunnybrook through any other entrance. Patients are encouraged to call Toronto Public Health first before coming to their assessment centre. Walk-ins are permitted. The hours of operation are Sunday to Thursday 9:00 a.m. to 5:00 p.m. For more information call the main line of the hospital at 416-480-6100. People with symptoms of fever, cough, sore throat, headaches, muscle aches, fatigue, runny nose and who work (or have close contacts who work) in the following settings should go to the assessment centre: in a long-term care facility, in complex continuing care, in a rehabilitation centre, in an acute care hospital, dialysis centre, retirement home, homeless shelter, prison or other settings with vulnerable populations. Other individuals from the community who are experiencing symptoms who should go to the assessment centre, include: those older than 60 who have heart disease, diabetes, lung disease, active cancer or are on dialysis. Only one person per household will be tested.

UHN Toronto Western Hospital (Dundas & Bathurst): An assessment centre has opened at Toronto Western hospital, located at 347 Bathurst St., on the northeast corner of Bathurst and Dundas St. Before coming to the Assessment Centre, the hospital is encouraging people to check the self-assessment tool at https://covid19toronto.ca or call Telehealth Ontario at 1-866-797-0000. An appointment is not required. The centre is open Monday to Friday 7:30 a.m. to 3:30 p.m. and Saturday and Sunday 10:00 a.m. to 6:00 p.m.

Unity – St. Joseph’s Health Centre (near Parkdale/High Park): An assessment centre is open on the first floor of the Sunnyside building. Enter from Sunnyside Avenue beside the parking lot. The hours of operation are 7 days a week from 8:00 a.m. to 4:00 p.m., or as earlier, as required depending on capacity and volumes. Warden Woods Community Centre – Temporary Site: An assessment centre is open at the Hub at Warden Woods Community Centre at 74 Fir valley Court in East York. Hours of operation are Wednesday, Friday, and Sunday from: 1:00 p.m. to 7:00 p.m. Please call 647-477-1640 to make an appointment. Walk-ins available for testing, vaccines and take-home test kit pick-up and drop-off. Registration ends at 6:45 p.m. – please arrive before this time to ensure you receive a test/vaccine.

Women’s College Hospital (downtown): A walk-in assessment centre is at the hospital, 76 Grenville Street. To get to the assessment centre, enter through the back entrance of the hospital. Hours of operation are Monday to Sunday from 9 am to 7:30 pm. You do not need an appointment but note this is not an urgent care centre – the only service being provided is COVID-19 assessment. GTA:

Ajax-Pickering: An assessment centre is open in a building adjacent to the Ajax-Pickering Hospital. (580 Harwood Avenue South) The hours of operation are Monday to Friday from 8 a.m. to 6 p.m. and Saturday to Sunday from 8 a.m. to 4 p.m. by appointment only. This location is not open for Walk-In appointments. To make an appointment fill out the assessment form on the public health website.

Burlington: An assessment centre has opened in Burlington at Joseph Brant Hospital, 1245 Lakeshore Blvd. The hours of operation are Monday – Sunday: 10:00 a.m. to 6:00 p.m. If you need to be tested, please call 905-632-3737, ext. 6550 to make an appointment.

Georgetown: An assessment centre is open at Georgetown hospital at 1 Princess Anne Dr., Georgetown, Ontario. Hours of operation are Wednesdays and Saturdays 8:00 a.m. to 4:00 p.m. Patients are seen by appointment only. Milton: An assessment centre is open at Milton District Hospital at 725 Bronte Street South, Milton. Oakville: An assessment centre is open at Trafalgar Memorial Hospital, 3001 Hospital Gate, Oakville. Hours of operation are Monday – Sunday: 8:00 a.m. to 4:00 p.m. Patients are seen by appointment only. To make an appointment call the Halton Health Care COVID-19 telephone hotline 905-203-7963.

Markham/Stouffville: An assessment centre is located at the Markham site of the Markham Stouffville hospital, located at 381 Church Street. The COVID-19 Assessment Centre cannot be accessed from inside of the hospital. The entrance is located on the south side of the hospital, east of the Emergency Department, see map. COVID-19 Assessment Centre parking is in lot #7. The hours of operation are 7 days a week from 9:00a.m. to 6:00p.m. The assessment centre provides screening, assessment and testing for individuals in Markham, Stouffville, Uxbridge and surrounding area who are experiencing possible symptoms of COVID-19. Before coming to the COVID-19 Assessment Centre, patients should call Telehealth 1-866-797-0000, York Region Public Health 1-877-464-9675, or their family doctor. They will advise on the best course of action, and whether or not to go to the Assessment Centre.

Newmarket: An assessment centre is open at Southlake Regional Health Centre, located at 474 Davis Drive in Newmarket. The assessment Centre located in the Ambulance Bay next to the Emergency Department. The hours of operation are 7 days a week, from 8:00a.m. to 9:30p.m. Please call Tel: 289-803-2199 to make an appointment.

 

Peel:

The Credit Valley Hospital: A drive-thru assessment centre is open at the hospital, located at 2200 Erin Mills Parkway. The Hours of operation are Monday to Sunday from 10:00 a.m. to 6:00 p.m.  For more information, call the main line of the hospital at 905-813-2200.

Mississauga: An assessment centre is open at Four Corners Health Centre located at 7205 Goreway Dr in Westwood Square, Mississauga ON. The hours of operation are Monday, Tuesday and Thursday from 12:00pm to 4:00pm, and Wednesday and Friday from 8:00am to 12:00pm. Walk-ins are prohibited. Patients can book an appointment by calling 416-312-0566.

Headwaters Health Care Centre: An assessment centre has opened at the health care centre in Orangeville, located at 100 Rolling Hills Drive. Hours: Monday, Wednesday and Friday from 10:00a.m. to 6:00p.m., Tuesday and Thursday from 12:00p.m. to 8:00p.m. Individuals 2 years of age and older can be tested. Walk-ins are accepted. Anyone with mild symptoms is being recommended to stay home and self-isolate. Directional signage and directional assistants will be available to direct drivers to the drive-through COVID-19 assessment centre. A member of the care team will assess them while they are still in the vehicle. Based on the initial assessment made by the care team, a decision will be made whether or not to test for the virus.

Richmond Hill: An assessment centre is open at Mackenzie Health at 10 Trench St., Richmond Hill. Walk-ins are prohibited. Use the provincial self-assessment tool first (available at: https://covid-19.ontario.ca/self-assessment/). If the self-assessment result advises getting tested for COVID-19, please schedule an appointment. To book an appointment call Mackenzie Health’s scheduling office at 905-883-1212 ext. 2004. The patient scheduling office is open Monday to Friday, 8:00 am to 5:00 pm and weekends between 8:00 am and 4:00 pm, excluding holidays. Hours: Monday – Friday: 10 a.m. to 9:30 p.m. and Saturday – Sunday: 10 a.m. to 5:30 p.m. The Assessment Centre has a separate entrance to the hospital between the B-wing and C-wing entrances. Rural Wellington: An assessment centre is opened at Legacy Groves, 235 Union Street East, Fergus. The hours of operation are 7 days a week from 10:00a.m. to 6:00p.m. Individuals 2 years of age and older can be tested. Walk-ins only, no appointments are required. Central Ontario/Grey Bruce/Muskokas:

Alliston: A drive-thru assessment centre is open at the Stevenson Memorial Hospital, located at 200 Fletcher Crescent. The assessment centre is located in the lower parking lot (staff/shipping & receiving entrance). The hours of operation are Monday, Wednesday, and Friday: 8 am to 12 pm. Patients arriving at the Assessment Centre will be asked to complete a self-assessment form.  Depending on symptoms, a nurse will determine if they should proceed for further assessment and testing or return home to self-monitor. You can also contact the assessment centre at (705) 435-3377.

Barrie: An assessment centre is open, located at the back of the Eagle Ridge complex at 490 Huronia Road, not directly at the urgent care clinic. Hours: Monday – Friday: 8 a.m. to 4:30 p.m. and Saturday – Sunday: closed. Patients are being assessed and referred for testing according to criteria. Walk-ins are accepted. Patients are asked to first do a self-assessment, under guidelines provided by the self-assessment tool that can be found here (https://covid-19.ontario.ca/self-assessment/#q0).

An assessment centre is open at Royal Victoria Regional Health Centre located at 29 Sperling Dr in Barrie ON. The hours of operation are Monday to Sunday 8:00 p.m. to 8 p.m. Walk-ins are prohibited. Patients can book an appointment by calling 705-797-3120.

Bracebridge: An assessment centre is open at the Rotary Centre for Youth on 131 Wellington St. Walk-ins are prohibited. The hours of operation are Monday – Friday: 9 a.m. to 5 p.m. Those who are experiencing COVID-19 symptoms should seek guidance from their family doctor as to whether they should go to the assessment centre. The family doctor’s office will be able to book an appointment for the patient if necessary. Those who do not have a family doctor can seek guidance from Telehealth Ontario, the Simcoe Muskoka District Health Unit or the Ministry of Health’s self-assessment tool. If it’s advised to attend a COVID-19 assessment centre, residents can call 1-888-383-7009 to book an appointment.

Collingwood: An assessment centre is open, located in a tent set up at the parking lot of the Erie. St. Medical Centre at 186 Erie Street. The centre is open from Monday to Friday: 8 a.m. to 6 p.m. Contact your family physician, nurse practitioner, Public Health, or use the Ministry of Health COVID-19 self-assessment tool (available at: https://covid-19.ontario.ca/self-assessment/) to determine if you require an appointment. To book an appointment, call the assessment centre at 705-444-5885 or get on the wait list by visiting www.gbfht.ca/cac/ and clicking on the “Get on Wait list” button, which can be accessed online until 7:30 p.m. every day.

Hanover: An assessment centre is located at the Hanover and District Hospital at 90 7th Avenue, Hanover. Appointments for the assessment centre will be posted up to 48 hours in advance only. Walk-ins are welcome but priority will be given to individuals with appointments. Hours: Monday – Friday: 12:00 p.m. to 6:00 p.m., with the last booked time slot at 5:00 p.m. Drive up to the main entrance of the hospital. A nurse will do a pre-assessment at the patient’s car. The assessment centre is screening patients who have COVID-19 symptoms “who are having difficulty managing at home”. For more information, call 519-364-2340. The COVID-19 assessment centre is not accepting walk-ins. People with COVID-19 symptoms should contact their Doctor or Nurse Practitioner, who will book an appointment if required.

Kincardine: An assessment centre is open at the Kincardine Hospital at 1199 Queen Street N, Kincardine. Hours: Monday to Friday from 10:00 am to 6:00 pm. The portable facility is located between the hospital and the medical clinic. No appointments necessary. The assessment centre is screening patients who have COVID-19 symptoms “who are having difficulty managing at home”. Maple: An assessment centre is open at 101-9401 Jane St in Maple ON. The hours of operation are from 11:30 a.m. to 6:30 p.m. Tuesday and Thursday.  Call 905-883-1212 ext 2004 to make an appointment.

Midland: As assessment centre is open in Midland by appointment only. The facility is located next to the Chigamik Community Health Centre at 845 King Street, Unit 3. The hours of operation are Sunday: 8 a.m. to 8 p.m.; Monday: 8 a.m. to 4 p.m.; Tuesday: 12 p.m. to 8 p.m. Wednesday: 8 a.m. to 4 p.m.; Friday: 8 a.m. to 4 p.m. and Saturday: 8 a.m. to 8 p.m. Walk-ins are prohibited. There are three ways to access an appointment with the assessment centre, including by visiting the Georgian Bay hospital’s website (http://gbgh.on.ca/covid-19-self-assessment-tool/), or by calling the assessment centre booking desk at 705-529-1025 or Ontario Telehealth at 1-866-797-0000. The booking desk is open seven days a week between the hours of 8:00 am to 8:00 pm.

Midland: An assessment centre is open at the Georgian Bay General Hospital at 1156 St Andrews Dr in Midland. The hours of operation are from 8:00 a.m. to 8:00 p.m Monday to Friday and from 8:00 a.m. to 4 p.m. on Saturday.  Call 705-529-1025 to make an appointment.

Mount Forest: An assessment centre is open at the Mount Forest Sportsplex, located at 850 Princess Street. Hours: Monday: 10 a.m. to 6 p.m.; Wednesday: 10 a.m. to 6 p.m. and Friday: 10 a.m. to 6 p.m. Walk-ins are accepted. However, in order to minimize wait times and protect the health of patients, the centre is encouraging patients to schedule an appointment. Call 519-323-0255 ext. 5082 to make an appointment.

Orillia: An assessment centre is open in the Kiwanis building on Orillia Soldiers’ Memorial Hospital campus, located at 170 Colborne Street West. Please enter the assessment centre on Volunteer Drive from Mississaga Street – Volunteer Drive has temporarily been designated as a one-way street. Hours: 11:00 am to 6:30 pm and 11:00 am to 2:30 pm on Saturdays and Sundays (the last patient will be seen at 2:30 pm). You will be assessed by a nurse from your vehicle to determine the next steps. The hospital is asking patients who think they have symptoms or have been in close contact with someone who has COVID-19, to please review the COVID-19 self-assessment tool to help determine if they need to come to the Assessment Centre.

Owen Sound: An assessment centre is open at Owen Sound Regional Hospital, located at 1800 8th Street E. The hours of operation are Monday to Friday: 12 p.m. (noon) to 8:00 p.m. The portable facility is just outside of the emergency department of the Owen Sound Hospital. Pre-booked appointments are required, and walk-ins are not being accepted. Please do not come to the assessment centre more than 10 minutes prior to your appointment. Parry Sound: An assessment centre is open at Parry Sound Mall on 70 Joseph Street, Unit 105 & 106. Patient visits at the assessment centre are by appointment only. Individuals displaying COVID-19 symptoms may call the COVID-19 Assessment Centre at 705-746-4540 ext. 5030 between the hours of 8 a.m. to 4 p.m., seven days per week.

 

Eastern Ontario and Southeastern Ontario, Peterborough and the Kawarthas:

Akausivik Inuit Family Health Team Assessment Centre: 24 Selkirk Street, Suite 300, Ottawa, ON, K1L 0A4. This centre provides culturally specific health care services for Inuit. Hours of operation are Monday – Friday: 9 a.m. to 5 p.m. For Inuit wishing to access COVID-19 assessment and testing, please call 613-740-0999 for an appointment. Services offered in Inuktitut and English.

Akwesasne: There is a mobile testing clinic for COVID-19 operated by the Mohawk Council of Akwesasne’s Department of Health. This testing clinic operates as a drive through site with appointments scheduled by the MCA’s Community Health Program (CHP). The hours of operation are Monday to Friday from 8 a.m. to 4 p.m. Testing is limited to individuals who hold an Ontario or Quebec Health Card, an Akwesasne Status Card and reside in the northern region of Akwesasne. If individuals who meet these criteria have symptoms of COVID-19, they can reach the CHP at 613-575-2341 ext. 3220 where they will speak with a CHP nurse to determine if they meet the criteria for testing. If the individual meets the criteria for testing, they will be given an appointment location, date and time where they will need to bring their health card AND their Akwesasne Status card or other government issued photo-ID.

Alexandria: An assessment centre is open at 62 Anik Street. The hours of operation from 8:30 a.m. to 6:00 p.m. and they are open Sunday, Tuesday and Thursday. Call 613-993-1375 or 1-800-267-7120 to make an appointment.

Almonte: An assessment centre is open on the corner of Spring Street and State Street in the brick building behind Almonte General Hospital Emergency Department on 75 Spring Street. The hours of operation are Monday to Sunday from 8 a.m. to 6 p.m. People seeking a test should call 613-325-1208 and leave a message. A COVID-19 Assessment Centre nurse will call back to book an appointment. There are no walk-ins.

Bancroft: An assessment centre has been opened in Bancroft at Quinte Healthcare Corporation North Hastings Hospital, 1 Manor Lane. The hours of operation are Monday – Friday: 10 a.m. to 6 p.m. Drive through testing is available. If you need to be tested, please call 613-332-2825 ext. 2222 to make an appointment.

Belleville: An assessment centre has been opened in Belleville at 45 College St. West. The hours of operation are Monday – Sunday: 9 a.m. to 5 p.m. Drive through testing is available. If you need to be tested, please call 613-961-5544 to make an appointment. Online pre-registering for COVID-19 assessment is also available at https://www.qhc.on.ca/pre-register-for-covid-assessment-centre-bellevilletrenton-only.php.

Brockville: An assessment centre is open at the Memorial Centre at 100 Magedoma Boulevard in Brockville. The hours of operation are Monday to Friday from 10 a.m. to 5:30 p.m. and Saturday to Sunday: 10 a.m. to 2:30 p.m. The centre is open for walk-ins. For more information, call (613) 704-1441.

Casselman: A drive-through assessment centre is open at 872 Prinicipale St in Casselman, the local office of the Eastern Ontario Health Unit. The testing centre will be open between 9 a.m. and 3 p.m. from Monday to Friday. The Eastern Ontario Health Unit is stressing that not everyone who is concerned that they may have COVID-19 should go to the assessment centre. Only people who have worsening, but not severe, COVID-19 symptoms (such as fever and worsening cough) should go to the centre for testing.

Cornwall: An assessment centre is open 850 McConnell Ave in Cornwall. This location is by appointment only and anyone wanting to book an appointment at the Cornwall testing centre should call 343-475-0160 between 8:30 a.m. and 3:30 p.m., Monday to Friday. Appointments can also be requested via email at covid.appointment@cornwallhospital.ca

Durham: An assessment centre is open at Lakeridge Health, located at Oshawa Hospital’s G1 unit. The hospital is located at 1 Hospital Court, in Oshawa. Use the entrance at the old emergency department bay on the south side of the building. Hours: 8:00 am to 8:00 pm from Monday and Tuesday, Wednesday 10am-6pm. Thursday to Sunday from 8am-4pm. Phone #: (905) 576-8711 The COVID-19 Assessment clinic is not open for walk in appointments, appointments can be made by physician referral, contacting Durham Region Health Department at 1800-841-2729 or by filling out an online assessment form at http://durhamcovidswab.lh.ca/. Another Assessment centre has been opened at the parking garage at 1 Mary St N in Oshawa Ontario.  Their hours of operation are from 10 a.m to 6 p.m. Monday to Friday.

Two new assessment centers are now open in Clarington (2440 Durham Regional Highway 2, Bowmanville) Hours of operation are Monday to Friday 8 a.m. to 4 p.m. and North Durham (19 Jeffery Street). Hours of operation are Monday to Thursday 12 p.m. to 8 p.m., Friday – Sunday 12 p.m.- 4 p.m.

Haliburton: A drive-through assessment centre is open in Haliburton County at the Haliburton Family Medical Centre, located at 7217 Gelert Street. The hours of operation are Monday from 9 a.m. to 1 p.m.; Tuesday: 9 a.m. to 1 p.m.; Thursday: 9 a.m. to 1 p.m. and Friday: 9 a.m. to 1 p.m. The Haliburton County COVID-19 Community Assessment Centre is staffed by health care providers who will complete a phone assessment and advise as to appropriate next steps, which may include self-monitoring instructions, self-isolation instructions, or further assessment and possible testing in the drive-through facility. Please note that testing is by appointment only. People with symptoms are advised to contact the Haliburton County COVID-19 Community Assessment Centre at 705-457-1212 (press 6).

Hawkesbury: An assessment centre is open in Hawkesbury at 750 Laurier Street. The centre is open Monday to Friday between the hours of 9:00 am to 5:00 pm. The centre is recommending people to call 911 or go to a hospital emergency if they are experiencing severe symptoms. If symptoms are mild, where there is no shortness of breath and symptoms can be managed at home, the centre is asking people to self-isolate for 14 days or until 24 hours after symptoms have fully recovered, whichever is longer. If symptoms are escalating but not severe, and you have either travelled outside of Canada or been in contact with someone who has been diagnosed with COVID-19 in the past 14 days, visit the assessment centre or call your primary health care provider. If both are closed, go to the hospital emergency department. If you are 70 years old or older and cannot get to the assessment centre, or have special considerations, call the Eastern Ontario Health Unit (EOHU) at 613-933-1375 or 1-800-267-7120.

Kemptville: An assessment centre is open at 15 Campus Dr in Kemptville ON. The hours of operation are from 9:00 a.m. to 5:00 p.m. Monday to Friday. Appointments are made in person at the hospital.

Kingston: An assessment centre is open at Beechgrove Complex (lies just south of the King St. West/Portsmouth Avenue intersection) Follow directional signage to Assessment Centre at 51 Heakes Lane Open 9 a.m. to 4 p.m. daily. Contact number (613) 548-3232. Before you visit the COVID-19 Assessment Centre, you should determine if you are a likely candidate for COVID-19 screening and testing. Use the COVID-19 self assessment tool to determine if you should go to the Assessment Centre or stay at home and self-isolate (the online tool can be found here: https://covid-19.ontario.ca/self-assessment/#q0).

Limoges: An assessment centre is open at 2015 Calypso St in Limoges ON. The hours of operation are from 9:00 a.m. to 4:00 p.m. Monday to Friday. Call 1-800-267-8260 to make an appointment.

Lindsay: A drive-thru assessment centre is open at the Ross Memorial Hospital at 10 Angeline St N, Monday to Friday from 9:30 a.m. to 5:30 p.m. Visitors are asked to enter from Kent St. and exit (right turn only) on Angeline St. N. It is recommended to call (705) 328-6217 and make an appointment before going.

Napanee: An assessment centre is open at the Lenadco Building at 310 Bridge St West in Napanee. It is open 7 days a week from 9:00 a.m. to 3:00 p.m. To make an appointment, please call the assessment centre at 613-354-8254.

Northumberland County: An assessment centre is open at the Northumberland Hills Hospital at 1000 DePalma Dr, adjacent to the Emergency Department. The clinic is open every day from 8 a.m. to 4 p.m. Assessments offered by appointment only. You can reach the assessment centre at 905-377-7783.

Orleans: An assessment centre is open at the Ray Friel Recreation Complex being run by the Montfort Hospital in Orleans. The hours of operation are from 8:00 a.m. to 3:30 p.m. Monday to Friday. Ottawa: An assessment centre is open at Brewer Park Arena, 151 Brewer Way, accessed from Bronson Avenue opposite Carleton University. The assessment centre is open Monday to Sunday from 8:30 to 8:00 pm. It is an out-of-hospital clinic where people will be assessed by a health-care provider and tested for COVID-19 if required. It is operated by The Ottawa Hospital (TOH) and CHEO, in partnership with Ottawa Public Health (OPH) and the City of Ottawa. They are telling patients to go to the Assessment Centre, not the Emergency Department, if: you have a new or worsening cough and/or fever and in the past 14 days have either travelled outside of Canada or been in contact with someone who has been diagnosed with COVID-19. Patients who meet the criteria will be tested. There is a second clinic open at the former D. Aubrey Moodie School on Moodie Drive, in Bells Corners (595 Moodie Dr, Nepean, ON). This clinic will treat patients with mild respiratory symptoms like cough, fever, and other cold-like symptoms, regardless of whether they meet the criteria to be swabbed for COVID-19. Hours of operation are 9am to 3:30pm Daily. The city of Ottawa has opened a third COVID-19 drive-thru assessment centre at RCGT Park at 300 Coventry Road. The drive-thru assessment centre will be open seven days a week from 11:30 a.m. to 6:30 p.m. Minors under the age 14 will NOT be tested at this location. Hospital Montfort – St. Patrick’s Intermediate School (Ottawa): An assessment centre has been opened at 1485 Heron Road, located in the same compound as the Federal Study Centre. The hours of operations are Monday to Friday from 9 a.m. to 3:30 p.m.

Peterborough: A mobile assessment centre is open at 2150 Ashburnham Drive in East City. The hours of operation are Monday to Friday from 10 am to 5:30 pm. Assessments are available by appointment only after completion of a self-assessment (https://covid-19.ontario.ca/self-assessment/#q0). Patients are being directed to call Peterborough Public Health at 705-743-1000 ext. 401 / 705-876-5086 to get a referral to the assessment centre if they meet the criteria.

Eganville has an assessment centre open at 178 Jane St on Friday September 4 from 2 p.m. to 4 p.m.

Douglas has an assessment centre open at 5390 Hwy 60 on Wednesday September 9 from 2 p.m. to 4 p.m.

Petawawa has an assessment centre open at 16 Civic Center Rd on Friday September 11 from 9 a.m. to 11 a.m.

Barry’s Bay has an assessment centre open at 65 Arena Rd on Friday September 11 from 2 p.m. to 4 p.m. Wednesday September 30 – 18 Billings St (11am-3pm)

Rockland: An assessment centre is open in Rockland at the Clarence-Rockland Family Health Team’s office, located at 2741 Chamberland Street. Simcoe: An assessment centre is open at 365 West St in Simcoe ON. The hours of operation are from 8:00 a.m. to 4:00 p.m. on Monday, Wednesday, Friday and Saturday. Please call 519-426-0130 ext 2234 to book an appointment.  Drive through testing is available at this location.

Smith Falls: An assessment centre is open at the Smith Falls site of the Perth and Smith Falls District Hospital (PSFDH), located at 60 Cornelia Street West. The hours of operation are Monday to Friday from 8 a.m. to 6 p.m. on Saturday: 10 a.m. to 4 p.m. The PSFDH says patients will need an appointment to be tested for COVID-19 at what will essentially be a drive-up clinic at its Smiths Falls site. Patients will be asked to drive up to the hospital’s main entrance on Elmsley Street North and wait for an in-vehicle assessment. Anyone can book their own appointment by calling 613-283-2330 ext. 1401 to schedule an appointment. The telephones are answered from Monday to Friday 8:00am–6:00pm and Saturday and Sunday 10:00am–6:00pm.

Trent Hills: An assessment centre is open at the Rotary Hall Boardroom at Campbellford Memorial Hospital on the basement level in the former paramedic bay and offices (146 Oliver Rd, Campbellford, ON). Access to the site will be a drive-through, drive-up centre. All visitors are to remain in their cars, and Assessment Centre staff will provide assessment/testing to you while you remain in your vehicle.

You can reach the assessment centre at 705-395-1801. The clinic is open Monday to Thursday from 9:00 a.m. to 5:00 p.m.

Trenton: There is an assessment centre open in Trenton at 8 Couch Crescent. The centre operates from Monday – Sunday: 9 a.m. to 5 p.m. Drive through testing is available. Please call 613-961-5544 to make an appointment.

Winchester: An assessment centre is open at the Lions Club Hall in Winchester, located at 515 Albert Street. Parking is at the back of the hall. There is signage to provide instructions for patients. Hours: 11 a.m. to 6 p.m. (with a lunch break from 2 p.m. to 2:45 p.m.) from Monday to Friday. For any more questions call the Eastern Ontario Health Unit at 613-933-1375 or 1-800-267-7120. Anyone with COVID-19 symptoms (such as fever and worsening cough), is first encouraged to go to the Ministry of Health’s website and complete a self-assessment online and follow the recommendations. You can also contact your primary care provider or call Telehealth Ontario at 1-866-797-0000. People with moderate, worsening (but non-severe) symptoms may be referred to the assessment centre.
